Telehealth Consent
Effective October 12, 2026
What telehealth is
Telehealth means receiving health care services from a licensed clinician using electronic communications, such as online questionnaires, secure messaging, and audio or video visits, instead of an in-person visit.
What you agree to
You consent to receive care through telehealth from independent licensed clinicians in the Finley Meds provider network.
You understand your clinician will rely on the information you provide, and you agree to provide complete and accurate health information, including all medications and conditions.
You understand your clinician may decide telehealth is not appropriate for you and may refer you to in-person care.
You understand that a prescription is not guaranteed and is issued only if your clinician decides it is medically appropriate.
Benefits and risks
Benefits include convenient access to care without travel. Risks include that information may be incomplete without a physical exam, technical failures may delay care, and, despite safeguards, electronic information could be accessed without authorization.
Telehealth is not for emergencies. If you have a medical emergency, call 911.
Your rights
You may withdraw this consent at any time by contacting us, without affecting your right to future care through other means. Your information is protected as described in our HIPAA Notice of Privacy Practices.
You have the right to see your clinician's name and credentials, and to access your medical records.
